The BP Texas City refinery came under fire after a 2005 explosion at the plant killed 15 people and injured 170, resulting in a felony plea deal and a heavy fine for the company. Now, the plant is once again under scrutiny after William Joseph Gracia, 56, was killed there in mid-January. Gracia, a grandfather of five, was planning to retire, according to his wife. He died January 14th after being struck in the head by a metal lid which blew off of a water filtration vessel when it was restarted. Federal investigators are currently trying to determine whether the accident was due to wear or corrosion of the bolts holding the vessel together.
